Filing a claim with Hydro‑Québec due to damage incurred
What you need to fill out the form
- Date of the event and the address where it occurred
- Claimant’s name, telephone number, address and, if claimant is a Hydro-Québec customer, their customer number

This nine-digit number beginning with 1 is indicated on the customer’s electricity bill.

- All supporting documents – photos of the damage incurred, bids, invoices, etc., along with any other documentation you feel is relevant to your claim. It is absolutely essential that these documents be enclosed with the claims form for your claim to be admissible.
